Requirements for a Valid DMCA Notice
Under 17 U.S.C. § 512(c)(3) a valid notice must include: signature of copyright owner, identification of copyrighted work, specific URLs of infringing content, your contact information, good faith belief statement, and a statement under penalty of perjury.
